Assessment and Case Management

Anyone may refer an individual to Florry Creative Care by contacting us directly at (813) 650-8242.

When a referral to Florry Creative Care is received our guest coordiator arranges to visit with the client and their family and together will determine the eligibility of service while conducting a complete assessment of the clients need. The coordinator will work with the individual and their family members to develop a plan to organize information and access other community resources.

Your personal coordinator will continue to work together with you to evaluate this plan of care on a regular basis.

If you are hospitalized and need home care for the first time, your personal coordinator will walk you through every step of the way.

Home Support
Personal Care Assistance
Home support workers assist with activities such as meals, minimal housekeeping and laundry and promote a safe environment. Home care attendants provide assistance with activities of daily living that may be required.This may also include helping with bathing, dressing and toileting.
